Medication Refill Requests

Please Read the Information Below Fully Before Filling out the Medication Refill Form

Please allow up to 3 business days to send the prescription to your preferred pharmacy. You will be charged $65 for refilling medications outside of appointment times.  Appointments are required at least every eight weeks for clients prescribed controlled substances. If you have an overdue payment, refills will not be released until payment is made. There are NO early refills for controlled substances, NO exceptions. If you are on several medications, try to request them all at the same time on the form to avoid separate fees for requesting medications individually. Medications can only be filled at Colorado, Maryland, Delaware, Vermont, Virginia, Florida, Oregon, New Mexico, and Connecticut pharmacies. PLEASE NOTE: There are several pharmacies experiencing an ADHD medication shortage or issues involving telehealth services (mainly CVS). If your current pharmacy is telling you they don’t have the medication available or will not fill it due to telehealth reasons, contact us ASAP. We can get the medication sent to another pharmacy in your area.
