After scheduling your appointment, you will receive an email instructing you to complete intake paperwork and consent forms. It is crucial that these forms are completed at least 24 hours before your appointment to avoid cancellation. There is a $150 fee for no-shows or late appointments. Please note that if you are approved for one of our ketamine treatment programs, you will be charged for the plan of your choice. Payment must be made before your medication is delivered.
